Dear Parents,

We wish to emphasize that the Southwest School Corporation does not provide any type of health or accident insurance for injuries incurred by your child at school.

Since children are particularly susceptible to injuries, we encourage you to review your present health and accident insurance program to determine if your coverage is adequate. If you do not feel your insurance is adequate because of a deductible or co-insurance clause, or if you do not have insurance, we encourage you to review the student insurance program. This plan will provide benefits for medical expenses incurred because of an accident.

The program is serviced by K & K Insurance Group, Inc., Ft. Wayne, IN.

There are several ways to obtain this coverage:

1. You may enroll online at www.studentinsurance-kk.com for quicker service
2. Pick up information from the secretary in your child's school
3. Download this form and mail the completed form along with payment as directed.  

DO NOT RETURN ANY ENROLLMENT FORM OR PAYMENT TO THE SCHOOL. Completed forms with payment are to be sent to K & K Insurance Group, P.O. Box 2338, Ft. Wayne, IN 46801-2338 or complete all information and pay on-line.
