This year Southwest School Corporation is asking parents to download many of the enrollment forms for the 2011-2012 school year from our website prior to enrollment. The purpose of this is to help parents get as many of the forms filled out before they come to the school for enrollment. Some of the forms are for just INFORMATION purposes only. You need not fill out anything, just download the forms. Some forms are for NEW students only. Other forms are for ALL students to fill out.    You will also get your students access to the harmony system. You can check grades, progress, attendance, pay book fees and cafeteria balances online, check on student internet access as well throughout the school year. We are asking parents/guardians to download and print out a copy of the forms they need and then bring the hard copies filled out with you to registration/enrollment in August. The informational forms are for you and do not need to be turned in to the school . Just bring the copies of forms you had to fill out to return to school.  It should speed up the enrollment process for you and make the enrollment easier in a shorter amount of time. Hopefully the lines will move quicker and involve less time for the parents during the registration process.

All enrollment forms must be done at the school during enrollment on August 2nd, 3rd, and 4th from 1:00 - 6:00 pm

 

Sullivan High School, Sullivan Middle School, and Carlisle Elementary Middle School encourages the use of the Harmony online student management system for web enrollment in August. Sullivan Elementary will be doing enrollment at the school only this year as online enrollment will not be available for SES.  For complete directions on using harmony to do online enrollment prior to coming to the schools in August for enrollment, please download the following directions from the link below. Please feel free to go ahead and do as much with online enrollment as possible ahead of coming to the school for enrollment. Many of our forms can be filled out in Harmony and submitted online. You can also get the paper copies of forms from the links above for your school

Parents be sure to upgrade your student's information for the 2011-2012 school year. Log in to Harmony using your student's web ID and password and click on the demographics link to the left. If you need a web ID please call your school or email harmony@swest.k12.in.us and request a web ID and password for each student enrolled at Southwest School Corporation.

 
 

Opportunities for School Choice for Students at Sullivan Elementary School

Title I schools that have not made Adequate Yearly Progress (AYP) for consecutive years are required to offer School Choice transfers to other corporation schools that are not in School Improvement and have adequate space to accommodate the students. If this is not possible, the corporation must seek alternative schools for School Choice from a neighboring school corporation, which may or may not be willing to accept interested students. The corporation is required to set-aside 20% of the Title I grant for School Choice and Supplemental Educational Services opportunities. In 2009-10, this amount was $65,727.69.

For the 2011-12 school year, Sullivan Elementary School students are eligible to request a School Choice transfer to Carlisle Elementary/Junior High School. Free Tutoring Opportunities for Students at Sullivan Elementary School

Sullivan Elementary School
students that are eligible for free or reduced lunch and are considered “non-proficient” based on ISTEP+ and DIBELS are eligible to enroll in a free tutoring program, known as Supplemental Educational Services (SES). This program allows eligible students to receive free tutoring and academic assistance in subjects such as Reading, English, and/or Math in effort to assist these students in improving their academic performance. These tutoring programs may be offered before or after the regular school day, on weekends, or in the summer. The corporation is required to set-aside 20% of the Title I grant for School Choice and Supplemental Educational Services opportunities. In 2010-11, this amount was $65,727.69. The Per Pupil Expenditure (PPE) amount allowed was $1,239.37.

In order to select the tutoring organization that will best meet your child’s needs, please review the list of state approved Supplemental Education Service Providers and a description of their services at the link below (not yet available). You may also request a paper copy of the Providers from your child’s school, or call the school principal for more information. All Providers available to eligible Sullivan Elementary School students are able to serve students with disabilities.  Currently, SES has an open enrollment period, and all providers are encouraged to use school space for tutoring sessions. To enroll your student, you may access an application at the link provided below or contact your child’s principal. 

In 2007-2008, 320 students were eligible to participate in SES at Sullivan Elementary School with 45 students participating.  In 2008-09,472 students were eligible to participate from Sullivan Elementary School and Carlisle Elementary/Junior High School, with 57 Southwest School Corporation students participating. In 2009-10, 140 students were eligible to participate from Sullivan Elementary School and Carlisle Elementary/Junior High School, with 26 students participating. If you would like for your child to receive these free services, please complete the SES Application and return it to your child's school office as soon as possible (not yet available).
